- Check with your health insurance provider to see if you are eligible for a reimbursement and follow their process for submitting the requested information.

- Complete the information below to request The Thoreau Club Membership Verification Form. We will mail this verification to you in 1-4 business days.

- Submit the insurance form(s) and the member verification form to your insurance provider and see the benefit!
